Records Request

REMEMBER, THIS IS YOUR DATABASE. A VERY COMPLETE AND ROBUST FOUNDATION HAS BEEN PROVIDED. HOWEVER, YOU MIGHT FIND THAT YOU NEED/WANT SOMETHING CHANGED. IN MOST CASES THAT CHANGE CAN BE MADE. THE FOLLOWING INFORMATION COVERS FEATURES OF THE “RECORDS REQUEST” TABLE THAT ARE NOT OBVIOUS AND SELF-EXPLANATORY.

Records Request

The Records Request table is accessed from the bottom of the SSA Case screen. The choices in the “Medical Provider” field are pulled from the Medical Providers table. You can save the documents received with a particular request in the “Documents Received” field as long as it is less than 15 mb.

An email alert will be sent to a designated email if, 10 days prior to a hearing, a record has been requested but not received. An email will be sent to the client whenever the “Request Date” or the “Date Submitted” fields are modified, with the name and phone number of the primary representative.
